Your Clearing Entry Form, available from the University and College Admissions Service (UCAS), is pre-printed with your unique clearing entry number, your personal details and any exam results UCAS has records of. No one else can use your form, so if you decide not to apply, just destroy it.

How To Fill In Your Clearing Entry Form (cef)
Section 1: UCAS' record of your examination results
In this section UCAS will have printed its record of your exam results (A level, AS, Scottish Highers/CSYS only). Check this carefully. If the results are incorrect or incomplete, write the correct results in this section and attach proof when you send off your CEF to a university. The universities and colleges will check any amended results with UCAS or the exam board or awarding body concerned.

Section 2: Applicant's telephone number(s)
Enter your telephone number and email address or fax number if you have one, so the university or college can contact you quickly.
Section 3: Name and telephone number of academic referee
Find out from your school or college which teacher - preferably your referee - will be available to discuss your application with admissions tutors during clearing, and enter the numbers where he or she can be contacted.
Section 4: Criminal convictions
You must state whether or not you have a criminal conviction (excluding a motoring offence for which a fine and/or up to three penalty points were imposed or a spent conviction). Tick either the 'Yes' or 'No' box. If you tick the 'Yes' box, a university or college may ask you for more details.
Section 5: Your choice of institution(s) and course(s)
Before you send off your CEF, enter the university or college code name and number, the course code, the campus code (if there is one) and the short form of the course title. You can find these details in the UCAS Handbook 2002 Entry or on the UCAS website at www.ucas.com
If you're happy for the university or college to consider you for courses other than the ones you've listed, say so in a covering letter when you send your form.
Section 6 and Section 7: Qualifications
Enter details of all exams you've taken, excluding A level, AS, Scottish Highers and CSYS. When you send off your CEF you can also enclose a covering letter and transcripts if you wish.
Section 6: Vocational qualifications
If your entry qualification is a BTEC National Award:
* Give the full title of any lower awards you hold, for example First or National Diploma or Certificate in Applied Biology.
* Give the full title of the highest award, for example Higher National Diploma in Applied Biology or National Diploma in Business Studies.
* Complete as requested.
* Enter the details of the units/modules/components of the highest award you have completed. If you can't fit all your BTEC results in Section 6, use the space in Section 7.
If your entry qualification is a SQA Vocational Qualification or SCOTVEC award:
* Leave blank.
* Enter SQA or SCOTVEC.
* Complete as requested.
* Enter component title, year completed and SQA or SCOTVEC catalogue number. If you can't fit all your results in Section 6, use the space in Section 7.
If your entry qualification is a GNVQ:
* Leave blank.
* Enter GNVQ.
* Complete as requested.
* First give the overall title of the qualification, using A for Advanced, I for Intermediate or F for Foundation. Next, in the 'Result' column, give the overall grade you achieved using P for Pass, M for Merit or D for Distinction. Then list the units you have completed. If you can't fit all your units in Section 6, use the space in Section 7.
Section 7: Qualifications not included elsewhere
Use this space for any other qualifications you have, for example GCSE, International Baccalaureate or overseas qualifications. Give the exact subject name used by the exam board or group in the relevant column. Use the following abbreviations for GCSE awarding bodies where appropriate:

If you have qualifications gained outside the United Kingdom, please give details of the exams you have taken for entry to higher education.
Section 8: Declaration
You must sign your CEF before sending it to a university or college. When you sign the form you agree that the information you provide is accurate, including any alterations you've made to Section 1. If you provide misleading or false information your application will be cancelled.
